The medical exam is required only once upon registration but if you have not yet registered then you have to watch for the expiry date, which is usually three months. If you failed to registered within the prescribed period, your medical exam expires and you will then need to perform another medical screening.

After that i want to make medical again or not?

@aarij There’s no need to attend the post-arrival medical screening again, if you have recently done the screening upon your arrival in Malaysia.
