Dear emgs officers
As I got the acceptance letter 4 July 2019 to study USM main campus. While filling the VISA application form there is a section of the health declaration, I want to know that I need to upload single page of health declaration which you have sent earlier or any other document along with health declaration form. Additionaly Do I have to send some document to USM after applying visa . Please suggest me .

  1.Please be advised that you have to download the Health Declaration form from the link below, fill it up and upload it upon creating the application:

  2. You are required to upload all the documents in the required fields upon creating application. Kindly liaise with your institution directly for information regarding supporting or additional documents required by your institution. Thank you.
