Dear Team, could you please explain this English on the HEALTH DECLARATION FORM: “ I hereby declare that I am free from the following diseases/conditions:
TB yes or no
Hep B yes or no

I think there is grammar confusion here
Please clarify
I don’t have any of the diseases me mentioned on the form.
So what answers should I pick: yes or no

  Please be advised that if you are free from all the diseases, you should tick “Yes”. If you are suffering from any of the diseases mentioned, you should “Tick” no on the specific disease. Thank you.
