I couldn’t load the image but if you mean the emgs medical check up form where they ask for vaccinations, for me the doctor refused to fill the form till I brought the official vaccination booklet where they wrote what they vaccinated me when I was born.. so I filled them up accordingly (I had two missing which aren’t a must)

So I would say it depends on the doctor who would sign your medical report , and whether it would be an issue for the immigration or not..
