So I think you need to work on this but Change in spelling is a lengthy and expensive process. So, It depends on you either you want to go with it or try your luck without that.

I have no Idea about that. You can try it.
I think is better for you to change name because if you go with that affidavit they might still reject it because it happened to one of my friend also
