I applied for visa on July 17 and received an email after 4 days saying issue in application with the declaration of accommodation so I submitted Google form from UM portal to give declaration of accommodation. As your Institution to send it on your behalf. Usually declaration of Accommodation is sent by the institution on behalf of the student.
Email your Institution, they will send it.
