Just as @abdulnuhu051 mentioned, the documents are different. The eVAL can only be issued upon approval of your visa application by the Immigration Department.

Attach it; with ‘goverment-supported medical certificate’ from government hospital or health center.

Clarify first by Reaching EMGS report that u did what were u asked for but what is the issue currently?

You are required to fill up the health declaration form as the required document to process your visa application. You may obtain the health declaration form from the link https://visa.educationmalaysia.gov.my/media/docs/Lampiran_B_-_Health_Declaration_Form_For_Applicant.pdf.
